Application
Follow these steps for downloading and filling out the application form:
1) Verify that you have Adobe Acrobat Reader (version
4.0 or later) installed on your computer. You can
download this free of charge at http://www.adobe.com/products/acrobat/readstep2.html
2) Download the DARTS application form by clicking
here. Depending
on the configuration of your web browser, the form will open in a second
web browser
window or you
will be prompted to save the file.
Before you start filling out the
form, you may want to print it so you know what information
you’ll
need to complete the form.
3) Move the "hand" pointer over a form box on the application. The "hand" should
turn into an "I-beam." The "I-beam" signifies a "fill-in" section
to the form. Click inside the box. You can now type into the box.
4) To move from field to field, use the Tab key. Shift + Tab will move
you to the previous field. Or use your cursor to click on the field
you want.
5) Fill out the form by typing text into the appropriate areas and checking
boxes where needed.
Please note that the information you entered isn’t verified
for you. Form fields simply allow you to type in information - you
must make sure that it’s correct.
6) Once the form is complete, print the form using
the "print" icon
in the Acrobat toolbar menu - not the print icon or command in your
web browser. Check the printed form very carefully for any errors,
fields that did not
print, or omissions. You must go back and make any changes in your
open document, and then reprint it — Adobe
Acrobat Reader doesn’t let you save the form once it’s
filled out, so you can’t go back and edit the form after you
close the window that shows the form.
7) Don’t close the window containing your filled out
form until you are certain the printed version is correct. Reminder:
Adobe Acrobat Reader
will NOT
allow you to save the completed forms.
8) Sign the application form
and either fax or mail it to DARTS (Attention: John Koenen.)
Fax to 651-234-2280; mail to DARTS, 1645 Marthaler Lane,
West St. Paul, MN 55118. Be sure to keep a paper copy for
your records.
